User Agreement

Welcome to StaminaSys, an innovative Electronic Health Records application (the "StaminaSys

Application" or the "Application"), web-based service, for the following "Users": Clinicians

(ex: osteopaths, massage therapists, naturopaths...), their non-clinical Staff (ex: secretary) and

their Clients (Patients).

If you are a client/patient of a therapist using StaminaSys, access to the functionalities of your record is free.
 

StaminaSys Application is operated by Integral Innovation Inc., incorporated under the Canada Business Corporations Act and is referred to as "we" or "us" in this User Agreement.

 

If you are a therapist, in order to access the features of the StaminaSys Application, you must first create an Account. Accounts are based on yearly plans. Your yearly plan can be paid either in one or twelve installments and will automatically be renewed every year through a payment from your credit card. To cancel your renewal you must notify us by email at: Support@StaminaSys.com

NB: ONE ACCOUNT PER USER ONLY
Do not share your account (username and password) with anybody for security and confidentiality purposes. Once you have created your account you will be able to: add as many members as needed from the ‘Team’ menu. Your therapist account also enables you to create an individual health record for each of your clients and securely manage, enter, view, store, and access his/her personal health information.

In order to create and use your Account, you will have to accept to be bound by the terms of use, privacy policy, copyright and disclaimer notices as mentioned here under.

In order to create and use your Account, you must agree to our terms and conditions by clicking “I Accept” and successfully complete the online registration and/or verification processes.

 

By registering and/or being verified and clicking "I Accept" at the end of this agreement, you acknowledge that you have read, understand, and agree to our terms of use and to be legally bound by them.
